SYNOPSIS

Team-based care is a model of healthcare delivery where a group of diverse health professionals work together to provide comprehensive, coordinated and patient-centered care. In this model, patients and care providers are equal partners.

A dedicated plenary at ICAM will explore how this collaborative approach can mitigate healthcare workforce challenges globally while leading to better outcomes for patients. Participants will hear from health professionals and patient partners about successful models of team-based care and education.

At the end of this session, participants will:

  1. Define the role HPE can play to optimize team based care;
  2. Describe how team based care can mitigate the human health resource crisis in Canada and globally;
  3. Identify how successful models of team-based education can lead to better patient care;
  4. Explain how the patient’s healthcare experience can be enhanced through health sciences education and team based care.
